Amputation Clinic Teams (ACT)

Amputation Clinic Teams (ACT) are facilities with a designated Amputation Clinic and dedicated interdisciplinary team. ACTS have a core Amputation Care Team, but may not provide the full scope of services available at a RAC or a PANS facility.

Facilities at this level may have an in-house Prosthetic/Orthotic Laboratory or an inpatient rehabilitation bed program; however for ACTs without an in-house lab, services are provided through referral to a PANS or through a contracted community-based prosthetic provider. Necessary inpatient rehabilitation services may be provided at the closest PANS or through an approved CARF accredited unit in the community.
